By submitting this form, you are giving your permission for this child to participate in all VBS activities and releasing Forest Hills Baptist Church and its staff from any liability. By enrolling your child, you are also giving the church permission to take pictures of your child for church promotions and publicity. (You May be asked to sign the official enrollment form when you drop your child off on the first day of VBS.)
